As a student attending co-op, I understand that I
must obey the Co-op Policies and Procedures.
Any adult may correct me if I do not, and they may notify my parents.

As a parent attending co-op, I have read the Co-op
Policies and Procedures and explained them to my children. I agree to all stated requirements, including
my responsibility to arrange for substitutes whenever I have to be absent. I understand that failure to comply with the
Policies and Procedures could result in the suspension of our membership in the
co-op, subject to review by the HEP Board and
Out of respect for the sponsoring church and fellow co-op members, I agree to teach all co-op classes in compliance with the teaching guidelines stated in the Policies and Procedures. I agree to adhere to the HHBC Belief Statement while teaching and interacting with children during all HEP activities. I understand that any teaching contradictory to the Belief Statement is grounds for removal of teaching privileges and/or membership by the Board.
If I ever have an issue or problem concerning the
co-op, I will try to discuss it with my Board member contact promptly.

families may have special needs. If you
